TERMS OF USE AND SERVICE
Confidentiality Policy
Any information that you share during session will be respected and protected in accordance to the below outlined confidentiality policy. The data collected will be stored in accordance to the principles lined out in the data protection act 1988 act and the GDPR.
All medical professionals, including your clinician are under legal obligation to share information with appropriate agencies (e.g., GP, the police, social services) if:
- We perceives that you are a danger to yourself or others
- We perceives that a minor is at risk
- We is required to share information under court order or by law
- We is required to disclose information for the protection, detection or prosecution of a crime
Every effort will be made to discuss such concerns and obligations with you before notifying relevant parties. In instances where this is not possible, he/she will use clinical judgement to act in the best conceivable way, to avoid causing any unnecessary harm.
Your clinician will be part-taking in regular supervision in accordance with professional standards and as a way of good practice, the confidentiality principles and duties extends to the supervisory relationship too.
Cancellation Policy
If you need to cancel your session we ask that you give us as much notice as possible, and at least 48h.
Records and Notes
Psychological therapy records and notes are confidential and are kept securely in accordance with the Data Protection 1988 Act. Records and notes are stored on a laptop which is password protected, the files are stored in a document management system which is encrypted and compliant with the General Data Protection Regulation (GDPR) statements. Notes will be kept for a period of 7 years after treatment has ended in line with my insurance requirements in case it may become necessary to share your data in the case of legal action following a complaint, or a legal duty by court order. After this period your notes and records will then be erased. Your contact, GP and medical details will be erased I month after psychological therapy has ended. Your phone number may be stored in your clinician’s mobile phone which is password protected, and your number will also be deleted within 1 month after treatment has ended.
Right to Access and Control your Personal Data
You have a right to access your personal data through a data subject access request, free of charge within 1 month of your request. You also have a right to transfer this data to another party.
You have a right to ask for information about you to be erased as long as it is no longer relevant to the original purpose it was collected. You can also request any inaccurate information about you to be amended. You can request a restriction on processing your personal data although this is only applicable in certain circumstances, usually for a limited time.
If you wish to withdraw your consent to my use of your data at any point please let me know in writing by letter or email, however data may need to be retained for lawful purposes in line with my insurance requirements in case it may become necessary to share your data in the case of legal action following a complaint, or a legal duty by court order.
